I plan on applying this year to the Interservice Physician Assistant Program. I (have) or (don't have) the prerequisite courses: (If you are missing courses, list them)...
I have (taken) or (haven't taken) the SAT. I (have) or (don't) have a minimum score of 450 in each section of the SAT.
I (require) or (don't) require a wavier. Waivers required are: example TIS, Age etc...
I am Active Duty, National Guard, or Reserve. (If NG or Reserve, you can apply for active duty training slots).
Please indicate which category your application falls in:
(1) Fully Qualified - I meet all of the requirements for the course and I don't require any waivers.
(2) Academic Delay - I meet most of the prerequisites, but I need to take a few extra courses this year to meet the requirements. I will take the following courses NLT 31 Dec 2012 to meet the requirement: ex... College Algebra
(3) RCC - I plan on applying for RCC to complete the following courses at Fort Sam prior to starting IPAP: ex. Chemistry I
(4) Partially Qualified - I meet all of the requirements except - Age, GT Score, SAT Requirement etc..
