Any application not submitted in its' entirety will be returned to the applicant without action.

Any applicant fully qualified/not selected for two consecutive boards will be considered not select/not competitive and may not reapply for one year from the date of the DA 61, IAW DA PAM 601-6, Ch2, para 6.

Applicants may request an exception to the one year wait period when more pertinent qualifications have been acquired. All requests must be routed through the applicants Company Commander or equivalent to the Warrant Officer Boards Branch OIC.

Applicants requiring the APFT waiver must have a profile and physical dated within 12 months of application submission.

New e-mail transmission that do not conform to this standard will experience significant delays or deleted from further processing
NOTE: When sending your packet, ensure the PDF attachment is no larger than 5 Megabytes (5MB) per e-mail transmission.

UPDATE - NCOs will submit their most recent TEN YEARS of NCOERs/Evaluation Reports/DA 1059s/ in lieu of the current 5 year requirement.
NCOs that have not been an NCO for that time-period will submit reports up to that point. EX: 8 years of NCO service will submit 8 years of evals; 6 year of NCO service will submit 6 years worth of evals and so on. Do not submit reports that extend beyond the current ten year period.

All applications must be sent digitally.

Prior to submitting your application please ensure you make a complete copy of your application.

Applicants twice not selected must send a complete new application after the one year wait period.

Only deployed personnel may mail their completed application however, electronic submission is preferred.
